The Humana Foundation, located in Louisville, KY, is focusing its grant making for 2017 in the following cities: Baton Rouge, La.; Broward County, Fla.; Jacksonville, Fla.; Knoxville, Tenn.; Louisville, Ky.; New Orleans; San Antonio; and Tampa Bay. The Foundation has chosen these locations because it recognizes opportunities to increase impact on health and well-being in these communities.
As the philanthropic arm of Humana Inc., the Humana Foundation seeks to improve community health and well-being through support of nonprofit partners that promote healthy behaviors, health education, and access to health services.
"The Humana Foundation looks forward to identifying outstanding nonprofit partners that are helping to reduce the impact of poor health behaviors, low health literacy, and lack of access to health services and support," said Virginia Kelly Judd, Executive Director of the Humana Foundation. "We are excited to hear from nonprofits about innovative and effective ways to improve health and well-being in the communities we serve."
In addition, current Foundation grantees (in communities noted above and beyond), that are receiving Humana Foundation funding in 2016, will have an opportunity to apply in the upcoming grant cycle.
The Humana Foundation will be requesting Letters of Intent (LOI) in December. The Foundation will accept LOI's from December 1 through December 15, 2016. Following a review of LOI's, selected organizations will be invited to submit online grant applications January 1 through January 31, 2017.
The Letter of Intent should be no more than two pages and briefly describe the following:
- Name organization, mission, and key successes to date
- Target population and community served: for example, geographic boundaries, age groups, ethnicity, disease state, income level, and other factors that illustrate the community. Clearly identify which of the following communities your program serves: Baton Rouge, Broward County in South Florida, Jacksonville, Knoxville, Louisville, New Orleans, San Antonio, and Tampa Bay
- Organization’s alignment to the Humana Foundation’s priorities and the intervention or prevention strategy that the organization will propose
- Demonstration of impact:
- Quantitative data including statistical analysis, if available
- Outcomes or results of evaluation of prior projects
- How the organization and the community will be positively impacted through support from the Humana Foundation

- Do not exceed two pages
- Submit in PDF format, double-spaced with 12-point font, 1-inch margins
